You have your unicorn arm, I have my swimming shoulder

Release date: 2016-11-03

The swimming shoulder is a general term used to describe a swimmer's shoulder injury caused by overuse of the shoulder. It is a chronic inflammation of the soft tissues of the shoulders (sputum, muscles, ligaments). This is also known as the Impingement Syndrome.

Take the freestyle of 25 yards as an example. If the right arm is to be drawn 8 times (pull water pull and arm recovery). Then swim 4000 yards and the right arm will rotate 1280 times. If your right shoulder is a little problem, after rotating 1280 times, you can imagine that this problem will become more serious.

What are the symptoms of swimming shoulders?

Symptoms vary depending on the cause. However, there are general rules to follow. The symptoms of backstroke are generally the most painful, while breaststroke is the least painful (unless biceps tendon is the main culprit). Freestyle is different depending on the stage of occurrence, and the diagnosis will be different (ie, if the pain is increased during the shifting phase, it may be a problem with the rotator cuff, and if the pain is increased when the water is initially pulled, it may be two The problem of biceps tendon. Sleeping on the injured shoulder will also aggravate the pain. When the problem worsens, it will hurt if you don't swim, and the shoulder will become painful when you touch it.

"Swim shoulder" is not an accurate diagnosis. In order to accurately diagnose (what muscles and tendons are injured), you should seek professional help from a sports medicine specialist (massage therapist, orthopedic surgeon). Subjects should have detailed swimming history and should understand the mechanics of swimming. This check includes, at a minimum, an assessment of the pain (weakness and weakness) of all your shoulders with and without resistance. Usually no X-ray is required.

What causes a swimming shoulder?

The reason will vary depending on the specific issue. If your problem is the most common syndrome, then this is usually related to muscle imbalance.

Freestyle uses the large "inner rotator" (pectoral muscle, broad back muscle) in the "pull stage" and the smaller and weaker "outer rotator" in the "shifting stage" (eg rotator cuff) Muscle group). Your problem may be one of the two, but the problem is usually in the weaker external rotator.

Some of the reasons that can cause pinch syndrome are:

(1) Insufficient body rotation during freestyle (should be 40°-60°). Insufficient rotation in the backstroke is more serious, which can lead to excessive pulling on the tendon.

(2) Excessively developed pectoral muscles, and underdeveloped rhomboid muscles, middle and upper trapezius muscles, levator scapulae, and upper dorsi muscle (resulting in instability of the shoulder).

(3) Weak rotator cuff muscle groups, these are dynamic stabilizers for the shoulders.

(4) Excessive internal rotation during "water intake": The middle finger should first enter the water, not the thumb and index finger.

(5) Excessive adduction in the “pull stage” (your hand should not cross the midline).

(6) If the internal rotator and adductor muscles (pectoral muscles, broad back muscles) are not strong enough, the elbow position is lowered.

(7) Excessive training and insufficient recovery time. In particular, a strict weight training and swimming program is carried out without giving the body enough time to recover. Overtraining also happens, for example, trying to make up for months of laziness in two weeks!

(8) Cantilevering on the non-breathing side, this phenomenon is common in unilateral breathing, meaning that when the air is ventilated, the non-breathing side of the arm does not have the elbow bending or the anchoring action, but the extension Hanging directly in the water will exacerbate or accelerate the pinch syndrome.

How to treat swimming shoulders?

The correct diagnosis (identification of 1. injured muscle and tendon and 2. stage and severity of the problem) is the most appropriate treatment. A good treatment plan includes:

(1) Apply ice to the shoulders for 20 minutes after training (if appropriate training) to reduce inflammation. Use anti-inflammatory drugs in the first few days. Long-term use of anti-inflammatory drugs may affect the stomach, kidneys and liver. It is not recommended to stop the pain and continue swimming.

(2) Rest and rest elastically depending on the severity of shoulder pain, which may mean complete rest (no swimming), or reduce the number of backstroke and butterfly strokes, or put your hands on both sides. At least two or three days after waiting for the shoulders, it is wise to resume swimming training.

(3) Care of professionals: may include ultrasound, interference wave electrotherapy, Cross friction massage, triggerpoint work (if trigger points are found during diagnosis), post-isometric relaxation of related muscles, Adjustment of the shoulder and / or neck.

(4) Rehabilitation exercise: strengthen weak muscles (usually the external rotation of the shoulder). These sports are not done in the gym. They are exercises with a small weight (3-5 pounds) under a specific and highly controlled level. If you have problems with shoulders, it is smart to do these exercises at least a few times a week. It is said that it usually takes one day of rehabilitation time to swim every day with a broken shoulder.

(5) Fix your training: Let the coach check your position (turning on the body) and the position of the arm. If there is no coach, ask a friend to record your swimming. The video often reveals the truth completely. Best: You can have a coach to record your swimming and watch your strokes with you. Don't wear a paddle swimming that is much bigger than your hand.

Xiaobian is troubled by swimming shoulders several times a year, at least two months at a time. According to the above-mentioned cause of self-examination, I feel that the shoulder is unstable due to the imbalance of chest and shoulder muscle development, and there is long-term training with paddles before the game. Pay more attention in the future. I hope to help you who are bothered by the swimming shoulders.
